New Patient Registration
We have around 13,552 registered patients. Our patient list size is currently to capacity; unfortunately we are not able to register any new patients at the moment, with the exception of relatives residing at the same address as a current patient of ours. Please telephone NHS England on 0300 311 2233 and they will allocate you accordingly.
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
We cover the following areas:
Broad Street Green, Cock Clarks, Cold Norton, Goldhanger, Great Totham, Heybridge, Langford, Latchington, Little Totham, Maldon, Mundon, North Fambridge, Northey Island, Osea Island, Purleigh, Stow Maries, Tolleshunt Major, Ulting, Wickham Bishops, Woodham Mortimer and Woodham Walter.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date.
This practice has a Patient Contract which all patients are required to sign. New patients who are on medication need to make an appointment to see a doctor for a medication review on registering.
Medical treatment is available from the date of registration. Please contact reception for further information.
Change of Personal Details
Please notify us of any change of name/address or telephone number. In an emergency this may be vital. Please make sure you include all relevant members of your household.
NB: If you move out of our practice area you will not be able to remain registered at this surgery, you will need to register at a surgery in your new area.
