Register as a Patient

Register with a GP surgery: Register with a GP surgery (gp-registration.nhs.uk)

We have approximately 12,500 registered patients. Blackwater Medical Centre is using Register with a GP for patient registrations. Residents across Essex can register with our surgery; however, if situated outside the specified areas, home visits and dispensary deliveries will not be available.

We cover the following areas:

  • Broad Street Green
  • Cock Clarks
  • Cold Norton
  • Goldhanger
  • Great Totham
  • Heybridge, Langford
  • Latchington
  • Little Totham
  • Maldon
  • Mundon
  • North Fambridge
  • Northey Island
  • Osea Island
  • Purleigh
  • Stow Maries
  • Tolleshunt Major
  • Ulting, Wickham Bishops
  • Woodham Mortimer
  • Woodham Walter

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

New patients just need to fill in an online form to get started. They do not need proof of address or immigration status, ID or an NHS number unless they would like online access to book appointments.

Paper forms are available for people who need them.

The service is designed and run by the NHS. It aims to cut practices’ workloads and make GP registration easier for the public.

Change of Personal Details

Please notify us of any change of name/address or telephone number. In an emergency this may be vital. Please make sure you include all relevant members of your household.

If you move out of our practice area you are able to remain registered at this surgery, however you will not be eligible for home visits or dispensing.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Non English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Access/Parking

Our main Practice in Princes Road (Maldon) is located close to the high street and adjacent to the council offices.

The branch surgery is located in Rowan Drive, Heybridge. Appointments for this site can be booked via the main Practice reception team.

Wheelchair access can be made through the front door and the toilets are suitable for disabled patients. Designated parking is available.

The practice has limited parking designated for patients' use whilst attending the surgery. Abuse of this facility causes congestion, which could delay emergency services. Additional parking facilities are at the public car park off Maldon High Street. Parking in the access road to the practice is limited to designated spaces.
